Nutrition Care & Patient Outcomes CEU Course Transitional Care and Nutrition Nutrition for Older Adults Webinar CEU Course

Course Objective

The purpose of this course is to raise the level of awareness among healthcare professionals of the far-reaching consequences of malnutrition on American hospital patients, and the impact that nutrition care has on maximizing patient outcomes while minimizing costs for healthcare stakeholders.

Approved Clock Hours

The program has been approved by the Commission for Case Manager Certification to provide CCM board-certified case managers with one clock hour.

Course Objective

The purpose of this course is to learn effective strategies to help patients to transition from an acute hospital stay to returning home or to another healthcare facility. The role of nutrition is key in executing a successful transitional care plan and is a contributing factor in reducing hospital readmission rates.

Course Objective

The purpose of this course is to increase awareness among healthcare professionals of the impact that the aging process has on the nutritional status of older adults, and the role that nutrition screening and developing evidence-based care plans have in improving nutritional status in frail and elderly adults.
